Date of Birth must be day/month/year. For example: November 1, 2005 should be entered as "01/11/2005" and June 30, 2001 should be entered as "30/06/2001"

For the Present please put a word or phrase as if it was in the sentence "Maybe it's the..." For example "bike", "pink doll you love".

For the Message please put a sentence that you would like to see as the PS. For example "Keep being good and go to bed early on Christmas Eve" or "I am really proud of your school work and how good you are at math."
